  8. _______________________, being of sound mind, willfully and  
  9. voluntarily make known my desire that my life shall not be  
  10. prolonged under the circumstances set forth below, and do 
  11. declare:

  13. 1. If at any time I should have a terminal condition as defined 
  14. and established in accordance with the procedures set forth 
  15. in paragraph 10 of Code Section 31-32-2 of the Official Code of 
  16. Georgia, I direct that the application of life-sustaining 
  17. procedures to my body be withheld or withdrawn and that I be  
  18. permitted to die;

  20. 2. In the absence of my ability to give directions regarding 
  21. the use of such life-sustaining procedures, it is my intention 
  22. that this living will shall be honored by my family and 
  23. physician(s) as the final expression of my legal right to refuse 
  24. medical or surgical treatment and accept the consequences from 
  25. such refusal;
